Heart and Stroke puts patients first

Journey of Vascular Patients and Families .JPGIn an effort to engage survivors in shaping the healthcare system and sharing their experience, the Heart and Stroke Foundation (HSF) created the Patient and Family Advisory Council (PFAC) – a group of vascular patients and their care partners.

The PFAC created two documents to offer insight from vascular patients and their care partners on their experience with the health care system, as well as opportunities for improvement of health care in Ontario.
